The Best Insurance Verification Form on the Web
We may be biased; however we’ve seen a lot of forms in our day.  This is the best Insurance Verification Form available anywhere, and we’re making it available to you here for free!  This version has our stamp of approval and has been custom designed by our staff to include all the little details that we’ve found to be critical to the process.

60 Day Collection Letter Template
90 Day Collection Letter Template
Collecting on outstanding balances can be a maddening process.  However, what you say (and how you say it) can make all the difference when sending collection letters.  These are the ones we’ve found to be most successful in collecting past due account balances.

Advanced Beneficiary Notice of Noncoverage
The latest version of the Advanced Beneficiary Notice of Noncoverage (ABN), with the release date of 3/2011 printed in the lower left hand corner, is available for immediate use.  Mandatory use of this version begins on January 1, 2012. All ABNs with the release date of 3/2008 that are issued on or after January 1, 2012 will be considered invalid.

CRM009-Non Covered Services Financial Disclosure Form
Since the Centers for Medicare & Medicaid Services (CMS) issued their official memo to all Medicare Advantage Organizations (MAOs, also known as Medicare Replacement or Medicare Complete plans), reiterating that advanced beneficiary notice of non-coverage forms (ABNs) are only applicable to Original Medicare beneficiaries, and not Medicare Advantage plan members, we’ve been recommending this form as a way to establish a process for holding plan members financially responsible for non-covered services. This is the Landmark non-covered services financial form. We suggest removing the landmark info and inserting your clinic’s info on it. We find this form is a good alternative to the ABN for Medicare Replacement Policies.
